Initial Psychiatric Evaluation
During your 45-minute initial psychiatric examination, we may inquire about the issues that are concerning you, and how they are affecting your life. We will also ask you about your symptoms and how you do to cope with them.
Follow Up Psychiatric Appointment
Follow-up appointments are used to make medication adjustments, start new medications, discontinue medications and fine-tune your treatment plan.
ESA Consultation/Paperwork
Emotional Support Animal Letter Consultation/Evaluations last approximately 30 minutes and consist of a detailed review of present symptoms, as well as past and current psychiatric and medical history. This evaluation includes patient and family medical and mental health history, as well as patient developmental, social, and academic history, followed by discussions of diagnoses (when applicable), and approval or denial of an ESA letter.
No-Show/Late Fee
We do understand that everyone has unplanned events that demand immediate attention and may require the cancellation of appointments. However, we ask that you understand that your booked time is set aside, especially for you and that when you cancel your appointment, that visit is a timeframe that another patient could have received the care and treatment they needed. Therefore, if you must cancel with less than 24- hour notice, you will be charged $60 for your missed appointment. Likewise, not showing up for a scheduled appointment will also incur a $60 no-show fee. If you are more than 15 mins late for an appointment, we reserve the right to reschedule your appointment for another time, and your appointment time may not be extended.
